This Contract between the State of Washington Department of Social and Health Services (DSHS) and the
Contractor is hereby amended as follows:
The dollar amount, $516,039, located in the Special Terms and Conditions of the Contract under Section 8.
Consideration, subsection b.- (3)2 is found to be in error and changed to read as follows: $267,270.
All other terms and conditions of this Contract remain in full force and effect.
