SUBRECPIENT AGREEMENT BETWEEN GftANT COUNTY AND HOUSING
AUTHORITY OF GRANT COUNTY SIGNED THE 20TH DAY OF JULY, 2021 AND
AMENDED APRIL 12,2022 1
Section 2, SERVICES PROVIDED BY TME SUB RECIPIENT is hereby amended to
include the following:
Subrecipient shall perform 'services necessary to provide CPI G eviction. prevention funds to
individuals/households in need of assistance. Services shall be performed per the CHG
guidelines.

Section 5, COMPENSATION is hereby amended as follows:
The maximum total amount payable by the COUNTY to the SUBRECIPIENT shall be increased
by $706,876.00 for a total of $2,015,33 1.00, per attached Amendments D & E, including
Amendment E Eviction, Prevention budget, tocontract #22-46108-08.
Housing Authority of Grant County has a UEI number of 095720462.
All other provisions of the original agreement or any modifications to the original agreement
remain in effect.
