ADDENDUM
EXTENSION OT CURRENT AGREEMENT
THIS ADDENDUM AGREEMENT, made July 1, 2020, is an Addendum to the Adams
County Vendor Agreement, entered into between Adams County Developmental Disabilities,
herchy known as Grant and Adams County Developmental Disabilities, and SL Start, LLC DBA
Compass Career Solutions ("VENDOR").
WHEREAS, Grant and Adams County Developmental Disabilities has agreed to assign the
Adams County Vendor Agreement to Compass Career Solutions LLC, an affiliate of SL Start, LLC,
which will be the new VENDOR of record.
WHEREAS, Grant and Adams County Developmental Disabilities and VENDOR
mutually agree to amend the terns of this agreement to June 30, 2021.
WHEREAS, Grant and Adams County Developmental Disabilities and VENDOR
mutually agree that all other terms and conditions set forth in that certain agreement, except duration
and VENDOR as set forth above, shall remain the same.
IN WITNESS WHEREOF, Grant and Adams County Developmental Disabilities and
VENDOR have executed this Addendum on the date first above written.
