ADDENDUM
CORRECTION OF EXHIBIT B REIMBURSEMENT RATES
THIS ADDENDUM AGREEMENT, made July 1, 2020, is an Addendum to the Grant
County Agreement, entered into between Grant County Developmental Disabilities, hereby known
as Grant and Adams County Developmental Disabilities, and Moses Lake Community Health
Center ("VENDOR").
WHEREAS, Grant and Adams County Developmental Disabilities and VENDOR
mutually agree to amend the contract to include the corrected reimbursement rate of $38,400 for
provision of Developmental Disabilities Community Education and Information Services (Parent
to Parent) in Section Exhibit II.
WHEREAS, Grant and Adams County Developmental Disabilities and VENDOR
mutually agree that all other terms and conditions set forth in that certain agreement, shall remain
the same.
IN WITNESS WHEREOF, Grant and Adams County Developmental Disabilities and
VENDOR have executed this Addendum on the date first above written.
