HomeMy WebLinkAboutAgreements/Contracts - GRISK19-137 Community Choice, dba Action Health Partners, Pathways Community HUB and Community Specialist Services Agency Memorandum of Understanding The purpose of this memorandum of understanding ("MOU") is to establish a working relationship for the provision of Pathways Community Specialist services while a formal -contract is being developed between Community Choice, d.b.a. Action Health Partners, ("HUB") and Grant Integrated Services, ("CSSA"). This MOU defines the roles of HUB and CSSA with respect to Pathways Community HUB Community Specialist services during 2019, the project start-up period.. It is agreed that HUB shall carry out the responsibilities of a Pathways Community HUB including, but not limited to: • Provide administrative services for CSSA, including access to the Pathways Community HUB Care Coordination System ("CCS"), CCS data platform, training of Pathways Community Specialists ("PCS"), receiving and assigning referrals and all other administrative and quality assurance activities required for successful operation of the Pathways Community HUB. • Match qualified referred. clients to CSSA following HUBs established and approved policies and procedures for assignment. • Provide all necessary and desired reporting to the North Central Accountable Community of Health (NCACH) and WA Health Care Authority, (HCA). • HUB will reimburse CSSA for successful completion of deliverables as set forth in Exhibit A- CSSA Payment Model and Monthly Deliverables. Pathways Community Specialist Reimbursement will be calculated per I FTE. • Collect, analyze, and report financial, and other performance measures to NCACH, Contracting Agencies, WA Health Care Authority and other stakeholder groups as appropriate. It is agreed that CSSA shall: • Provide Pathways Community Specialist services to clients assigned to CSSA by the HUB in accordance with established Pathways methodology, policies and procedures which can be found in the Action Health Partners Pathways Community HUB General Operating Policies and Procedures Manual. • Work in partnership with HUB to develop business practices that will ensure successful client engagement in order to reach full PCS caseloads as soon as possible of first PCS client assignment. • Maintain adequate separation between duties/activities associated with Pathways Community HUB and other services provided by CSSA in order to maintain strict adherence to Pathways methodology policies and procedures, support information gathering for reporting and' monitoring of quality and to ensure PCSs receive and maintain all needed training and support as detailed in the policy entitled "Boundaries, Ethics and Training Policy and Procedure" found in the General Operating Policies and Procedures Manual referenced above. Use Exhibit B to account for PCS FTE time as outlined in the deliverables in Exhibit A. Account separately for the expenses and revenues associated with Pathways Community Specialist Services versus other programs of CSSA in order to support accurate discussions with the HUB regarding payment levels and financial sustainability of the Pathways Community Specialist program. It is agreed by both parties that: • A strong collaborative relationship with respect to the delivery of Pathways Community Specialist services is highly desired. CSSA MOU Page 1 of 2 Action Health Partners Pathways Community HUB • Should an issue arise that is not reflected in this MOU, both parties shall communicate by appropriate means to resolve the issues so as to best support the Pathways Community Spceilalist clients or staff involved. Further, they will use best efforts to execute an Action Health Partners Pathways Community HUB Community Specialist Service Agency Agreement as soon as possible, Client's medical data shall be communicated subject to HIPAA guidelines, the Action Health Panners Pathways Community HUB FIIPAA Compliance Policies and Procedures Manual, (to be provided to CSSA), and to the attached Action Health Partners Pathways Community HUB BLIsiness Associate Agreement. This MOM shall remain in force until December 3 1, 2019 or it is replaced by an Action Health Partners pathways COMMUllity I IUB COITIML111ity Specialist Service Agency Agreement, whichever is sooner, Revisions many be made at any time by nILItLial written agreennent of the parties.
