GENERAL BUDGET PROVISIONS to read as follows: D. Planned Rates information will correlate with the actual service hours that clients receive per the Person Centered Service Plan. Section VII. REVIEW AND EVALUATION, to read as follows: Quality Assurance and Service Evaluation: The Vendor shall develop and have available an evaluation system to review services. The evaluation system must have both a Quality Assurance and a Quality Improvement component, and both must include objective measures. The objective measures at a minimum will include performance indicators, by acuity, that include the number of new jobs; job loss and why — retention rates; the percentage employed earning minimum wage or better, and the average number of hours worked. A copy of such evaluation system shall be provided upon request to County for review and approval. Replacements to Exhibit A: • Section XV. INCIDENT AND ABUSE REPORTING A. to read as follows: A. Incidents involving injury, health or safety issues are immediately reported to DDA Central Office and the County, by the next working day in accordance with DDA Policy 6.08 Incident Management and Reporting Requirements for County and County Contracted Providers. Additions to Exhibit B: • Section 3 INDIVIDUAL SUPPORTED EMPLOYMENT, to read as follows: k. Clients enrolled in Individual Employment, Group Supported Employment or Prevocational services who have not obtained employment after six (6) months, the vendor will assure the following steps are taken: • Review of progress towards employment goals; • Provide evidence of consultation with family/client; and • Develop of additional strategies with family/client, County, employment staff, and CRM. There is documentation of these strategies. • Section VI STATEMENT OF WORK, R, to read as follows: R. The County has policies, procedures, and practices in place that clearly define responsibility, and the relationship with the Vendor. • Section VI STATEMENT OF WORK, N, under Grant County shall, to read as follows: S. Review the Vendor for compliance with the following policies and procedures: • Confidentiality • Safeguarding Personal Information • Debarment Certification • Dispute Resolution • Proof of Insurance Liability • Maintenance of Records Section VI STATEMENT OF WORK, L, under the title of: The Vendor shall, and will be in addition to the existing section L, to read as follows: Vendor will ensure that it is a qualified provider and must be a county or an agency/individual who holds a current contract with DDA or the County. Approval of fees is the responsibility of the Developmental Disabilities Association (DDA). ii. Grant County Developmental Disabilities' current hourly rates are set at: 1. Individual Employment services - $70.00 per hour 2. Group Supported Employment services - $65.00 per hour 3. Community Inclusion services - $35.00 per hour Section VI. STATEMENT OF WORK 1, to read as follows: 1. If after twelve (12) months the client remains unemployed, an additional review will be conducted. The provider will address steps outlined in the previous six month progress report in the next six month progress report. The client may request to participate in Community Inclusion (CI) activities, or the client may choose to remain in an employment program. When requesting to participate in Cl, the client shall communicate directly with his or her DDA Case Manager. The DDA Case Manager is responsible for authorizing CI services. Section VI.
