Thank you for your commitment in supporting Grant Integrated Services while we've partnered in
providing Pathways Community Hub services. We've appreciated the training, coordination, and
opportunities to provide these services to our community, however, we have also encountered barriers
making it difficult to continue to effectively provide this service. So we have decided, not to renew this
Contract/MOU after December 31, 2019. Please accept this letter as our 30 days' notice.

Good afternoon,
I have the below MOU tickled for every January to make sure it is still working appropriately. Can you please confirm?
Also, if you would like me to confirm something different than every year, please let me know.
Memorandum of Understanding (MOU) for the provision of Pathways Community Specialist services between
Community Choice, d.b.a. Action Health Partners (HUB) and Grant Integrated Services. The MOU will remain in force
until such time as revision is deemed necessary or it is replaced by an Action Health Partners Pathways Community HUB
Community Specialist Service Agreement.
