The Chair of the Board was authorized to sign an order form from Credible Behavioral Health, Inc. for
add-on billing training consulting services with Grant Integrated Services and a one-time additional fee of
$6,800.00. This is an addition to the Letter of Intent which adds this service to their Agency's existing
agreement.

Dear Mr. Taylor:
As an addition to Grant Integrated Services' ("Agency") Letter of Intent ("LOI"), Credible Behavioral Health, Inc.
("Credible") adds the following services to Agency's existing agreement:
Billing Training Consulting Services
• Credible agrees to provide up to two (2) days of onsite billing training to review and clarify all elements of
Credible's standard billing process with Agency.
• Credible also agrees to conduct a two (2) hour month end close remote training forty-five (45) days after the
billing training occurs.
• Consulting will be conducted at a mutually agreed upon date and time and during normal business hours of
8:30am — 5:00pm in Agency's specific time zone.
• Agency understands that any consulting services provided over the initial two (2) days will result in additional
fees.
• The one-time fee associated with this service is eight thousand ($8,000) dollars.
o In the spirit of Partnership, Credible is providing a fifteen percent (15%) discount for a revised one-
time fee of six thousand eight hundred ($6,800) dollars.
• Agency acknowledges that these consulting services listed in this Order Form are only for Agency and
Agency's personnel purposes.
• Agency understands that the application of any recommendations made during these Consulting Services will
solely be applied to Agency.
• Agency will be invoiced seventy-five percent (75%) of the one-time fee upon signing this Order Form. Agency
acknowledges that seventy-five percent (75%) of the one-time fee will be due and must be paid in full in order
for Credible to start the engagement. Agency will be invoiced the remaining twenty-five percent (25%) of the
one-time fee upon the completion of the engagement.
• Agency acknowledges that Agency's account with Credible must be in good standing prior to Credible's
commencement of this service.
General Terms
This Order Form is in addition to the existing executed LOI and/or Business Services Agreement ("BSA") and does not
supersede such LOI and/or BSA. All invoices due upon receipt.
