This is an emergency request for out of state travel to enable a Community Living Support Aide to provide
transportation and travel support for a Directions in Community Living client for medical reasons.
Our client, 685701, is experiencing a serious medical condition and the specialist has recommended that
client have hospitalized specialty treatment at an out of state hospital (Portland, Oregon). The current
specialist has indicated that the condition is serious in nature and rather emergent so travel would be today,
June 28, 2019 thru possibly a short hospital stay July 5, 2019.
Directions in Community Living has sufficient funding for this transportation and hospital support service. If
you have questions or concerns about this travel, please call me at extension 5434.
