1. Possible vouchers for insurance, legal services, grant administration, etc.
/ 2. Washington State Health Care Authority, Contract Amendment, Prevention Services, through
I/ Grant Integrated Services, Contract No. 1763-94250, Amendment No. 4. This amendment
increases the agreement in the amount of $2,300.000 for a total contract amount of
$298,127.00. Term of the contract is also extended to September 30, 2019.
/ 3. Signature Authorization on Amendment A to Washington State Military Department Contract
No. E19-019. The amendment serves to increase the contract amount by $4,173 for a new grant
total of $48,973. Funding authority is the Washington State Military Department and State
Enhanced 911 Funds. ymkd C. 1
4. Amended Reimbursement Request (A-19) for contract work performed by Grant Integrated
Services' Prevention and Recovery Center (PARC), during December, 2018 in the amount of
$8,068.54 on Washington State Department of Social & Health Services Health Care Authority
Contract No. 1763-94250.
