The HWS has had many discussion regarding implementing a Wellness program at Grant County and developed the following outline to kick start the first year: • Open to employees only • To earn the incentive, the employee must complete 4 of the following G activities: i ) Annual Physical Exam 2) Biometric Screening 3) Personal Health Assessment (PHA) -this is a questionnaire members would fill out online at the HMA Wellness Portal. At the end of the PHA the employee receives a personal health report with wellness tips and education 4) Preventative vision exam 5) Preventative dental exam 6) Tobacco Cessation Program -non-tobacco users fill out an affidavit stating they do not use, and tobacco users sign up for the cessation program • Incentive is $ 50.00 (less payroll taxes) for completion of 4 of the G activities. No pro -rating would be allowed. • All activities must be completed by November 15th, 2019 -credit for activities already achieved in the 2019 year will be applied. • Incentive payment to be received in JQx�war-y2020 6%� +44W 4L*It,-OL� L "4 At the August monthly meeting, the HWC discussed these results and responded to an email ballot on 9/19/2019 and passed. Therefore, the HWC would like to recommend that Grant County implement the wellness program outlined above with further discussion and detail to be approved in 2020 for the continuation of the program.
