REQUEST TO THE COUNTY ROAD DEPARTMENT
FOR REIMBURSABLE WOI��&,
To The County Engineer:
Pursuant to terms of an agreement between the County and City Of QUiDcY
adopted. and approved on June 25, 1976 the undersigned. hereby requests the County to
perform the work listed below at a time and in a manner convenient to the County. The undersigned
further requests that the work be completed no later than December 31, 2019
and hereby guarantees reimbursement to the County for all work done -up to a maximum. of $ 2,,000.00
SHV load rating calculations performed on Br -345 on Rd R -NW. Also includes installation of load
posting signs if necessary.
All terms of said agreement shall apply with the following exceptions:
