This contract is for the personal services of Matt Bellace, Ph.D. on the engagement below and is made this 11'h
day of February 2019 between the undersigned sponsor of entertainment (herein called "SPONSOR") and the
undersigned ARTIST.

SPECIAL TERMS OR CONDITIONS:
• Up to a total of 8 programs, as discussed with Bernadette Crawford at Grant Integrated Services
• Travel expenses included
• $4500 deposit due 30 days after the SPONSORS signed date below
• Remainder of payment ($4500) due on or before the date of the program

No performance shall be recorded, reproduced, or transmitted from the place of performance without verbal or
written permission from the ARTIST/SPONSOR involved.
If agreement is canceled by SPONSOR and the date is not resold, the following percentage of the fee will be in
effect from the time of notification to ARTIST: 14 days or less prior to scheduled engagement= 100% plus
airline penalties incurred by ARTIST, 30 days or more= 50% plus airline penalties. If the date is resold, deposit
can be applied to future booking, within a year.
If ARTIST cancels due to unforeseen personal emergency or airline/ weather complications, ARTIST will make
the best effort to reschedule the date, at a date mutually acceptable to both Artist and Sponsor during the current
academic year using the same terms of this agreement. If neither of these conditions can be met, then ARTIST
will immediately refund the Deposit.
If the engagement is canceled due to an ACT of GOD (example: ice storm, earthquake) or dangerous situation
(example: riots, violence), SPONSOR will be responsible for travel expenses by ARTIST. ARTIST's fee will
be charged in full and SPONSOR will have up to one year to reschedule the event for no extra charge. If
ARTIST has commenced travel to the engagement, airline penalties may apply to reschedule flight.
Your signed copy of this Agreement is acknowledgement that the ARTIST has accepted this engagement and
has agreed to appear at the time and place specified above. To confirm your agreement of this contract please
sign and return to the address below.
