Re: Unpaid Self -Pay Insurance Premiums — Amanda Oliphant
Amanda Oliphant is a former Grant County employee who worked as a Community Living Support
Aide, PT with the Renew - DCL from July 10, 2017, through November 23, 2021, when she
voluntarily resigned for personal reasons.
While employed with Grant County Amanda moved into leave without pay status during pay periods
11/10/2021 and 11/24/2021. Amanda was sent self -pay premium notices on 11/24/21, 12/10/21,
12/23/21, 2/1122, 2/10/22, and 2/25/22 with a Final Notice mailed on 3/10/22, and owes the health
plan $183.56 in unpaid leave without pay premium for core and voluntary benefits.
