10. DESCRIPTION/JUSTIFICATION OF AMENDMENT:
This amendment adjusts the funds in the Budget Sheet (ATTACHMENT E) based on updated
expenditures, also to include contract modification.
11. AMENDMENT TERMS AND CONDITIONS:
1. Contract expiration date August 15, 2021 remains unchanged as described on page 2 of this amendment.
2. Contract performance period of July 1, 2020 through June 30, 2021 remains unchanged as described on page 2 of
this amendment.
3. Change the overall FY21 funding amount from $44,687 to $53,335; an increase of $8,648 as described on page 2 of
this amendment.
4. Replace Original Budget Sheet Attachment E with attached Revised Budget Sheet Attachment E.
5. Change the FY21 CPD6 funding amount from $0 to $8,648; an increase of $8,648 as described on page 2 of this
amendment.
This Amendment is incorporated in and made a part of the contract. Except as amended herein, all other terms
and conditions of the contract remain in full force and effect. Any reference in the original contract or an
Amendment to the "contract" shall mean "contract as amended". The Department and Contractor acknowledge
and accept the terms of this Amendment as identified above, effective on the final date of execution below. By
signing this Amendment, the signatories warrant they have the authority to execute this Amendment.

Here is Contract E21-018 Amendment A -Grant County SECO CPD FY21
This is the same contract that I sent the new Signature Authorization Form over for last week.
(Sorry -If I would have known this was coming, I would have sent them together.)
The Amendment A is for an additional $8,648 for our 911 phone system upgrade we are in the middle of.
The funds -along with all the CPD funds are available to MACC on a reimbursement basis.
You should have a copy of the original contract but I have attached it here as well for your information.

ELECTRONIC SIGNATURE: If signing with electronic signature, ensure the proper person is signing the
contract amendment and then return via email to Contracts.Office@mil.wa.gov and cc
Teresa. Lewis@miLwa.gov --- Electronic signature refers to a scanned copy of the subrecipient's wet
signature. NOTE: we cannot accept a digital signature —a digital signature is a signature that is created via an
application such as DocuSien or Adobe Pro that is not a scan of an actual signature. In order for the Military
Department to be able to accept "digital signatures", there has to be an approved policy in place that spells out
the authentication parameters, etc.
o The Military Department does not currently have such a policy, therefore we are not currently able to
accept "digital signatures" on agreements and/or amendments.
• HARD COPY SIGNATURE (ORIGINAL): Print the attached document (ensuring that it is single sided
printed), have the appropriate person sign the contract amendment. If you want a hardcopy sent back
to you through the US Mail, you must submit two signed copies with a note requesting an original to
be sent back to you. Send the signed copy(ies) to the following address:
