FOR REIMBURSABLE WORK

Pursuant to terms of an agreement between the County and Franklin County

adopted and approved on the undersigned hereby requests the
County to perform the work listed below at a time and in a manner convenient to the County. The
undersigned further requests that the work be completed no later than 09-03-21
and hereby guarantees reimbursement to the County for all work done, including a 10% Administration Fee
for up to a maximum of $ $35,000.00
Work to be Completed:
Provide chip spreader with operator to chip seal approximately 45 miles.

Authorization to purchase:
Public Works:
Serial No. 21.059
Public Works is seeking authorization for the purchase of dump truck tires through Tire Factory,
Omnia contract number #2019209,, for Central Shop stock in the amount of $5632.45.
These tires are regular items for Central Shop stock for use on Public Works dump trucks.
I have reviewed the information and find it acceptable to move forward with the purchase of
these items.
