Re: Insurance Representative
This memo is in response to your discussions regarding the County insurance
representative. Scott Davis of Martin Morris Agency Inc. of Ephrata is our current
representative. Shane Heston of Robert Trask Agency recently approached the
BOCC asking for an opportunity to be the County's insurance representative for
the WRCIP policy.
Each of you has recently spoken with both Scott Davis and Shane Heston. Please
indicate below which agency you want to utilize as Grant County's WRCIP
insurance representative and the effective date, if a change is made.
Martin Morris Agency, Inc.
Scott Davis
Trask Agency, Inc.
Shane Heston
Effective Date
If a change is made, the attached Agent of Record Change form will also need to
be signed.
