Re: Unpaid Self -pay Insurance Premiums — Stacy Sanislo
Stacy Soni to is a former Grant County employee who worded as a Corrections R.N with the
Sheriff's Department from August 10, 2017 through March 12, 2021 when she voluntarily resigned
for personal reasons.
While employed with Grant County Stacy moved into leve without pay status during pay periods
2/26121, 3112/21. and 3/261 1. Stacy paid her portion of the premium due to the health plan for pay
period 2126121, but to -date has not paid for pay periods 3/12/21 and. 3/26121.
Stacy was sent self -pay premium notices on 2126/21, 31121.21, 3126/21, 419121, 4/231211 51101211
5121121, and 614121, with a Final Notice mailed on 716121, and owes the health plea $517.46 in
unpaid leave without pay premium.
