Philumina Johanni is a former Grant County employee who worked as a Trainee — Post Bar with
the Prosecutor's Office from April 1, 2021, through October 8, 2021, when she voluntarily resigned
for, personal reasons.
While employed with Grant County Phillumina moved into leave without pay status during pay
periods 8/25/21 and 9/10/21. Philumina wassent, self -pay premium notices on 9/10/21, 9/24/21

on 3/10/22 and has had *rnultiple follow up outreaches through phone, calls and email following an
initial agreement, that she would remit payment upon her separation from Grant County 1ni 1. and again in
April of 2022. Philumina paid for pay period 8/25/21 and owes the health plan the remaining
balance of $30,38 in unpaid leave without pay premiurn for, core and voluntary benefits for pay

Sean Mat ig is a former Grant County employee, who worked as a Truck Driver with Public
Works from May I Ot 2021, through April 7, 2022, when he was separated from Grant County due to
not passing his probationary period.
While employed with Grant County Sean went into leave without pay status during pay periods
10/22121 and 11,15/211 following his last day of work on September 27, 2021. Per policy, Sean was
transitioned to COBRA with active coverage ending 9/30/21 and was sent self pay premium notices
for the, transition period 10/22121, 11/5/21 t 111/19121, 12/7/21112/171211 2111/221 2/10/22, 2/25/22,
3/11/22, 3/25/22, 418/22, 4/25/22, with a Final Notice mailed on 6/17/22. Sean owes the health plan
$2295.00 in unpaid leave without pay premium for, core benefits,
