Pay New Hope an additional $3,057 in flexible financial assistance. This brings the total payment under this contract to $57,057. It is entered into a spirit of good will on the part of both parties on October 15, 2020 and extends through September 30, 2022. (FUND SOURCE: CFDA# 16.034) WSCADV is retaining the Contractor to participate in a program of the Washington State Department of Commerce., Community Services and Housing Division - Office of Crime Victims Advocacy (OCVA) Coronavirus Emergency Supplemental Fund (CESF) to implement an innovative partnership with Columbia Basin Health Association (CBHA) - Parents As Teachers home visiting program. The domestic violence advocates of the Contractor will provide mobile advocacy, community engagement, resources,, networking and Domestic Violence Housing First (DVHF) flexible financial assistance to pregnant and parenting survivors and their families who are enrolled in the CBHA - Parents As Teachers home visiting program for WSCADV. Flexible financial assistance is used in the Domestic Violence Housing First approach to support survivors of domestic violence and their children with specific housing and service needs as defined by the survivor. Flexible financial assistance may be dispersed as a payment on their behalf to a landlord (i.e., for rent and/or deposit), childcare provider (e.g., for childcare expenses so that the survivor may pursue employment, or court fees/fines), or other kinds of debt. Contractor is encouraged to use flexible financial assistance in whatever way survivors identify will best support their stability and/or to obtain and retain housing. This includes not just rent and utilities, but also childcare and any child need, legal assistance, and transportation (including car repairs and gas vouchers), work uniforms, back debt, or the like, that support employment and housing. There are no service or income requirements required for survivors to receive flexible financial assistance. WSCADV further expects the Contractor to offer domestic violence advocacy support as the survivor defines and needs. By this,. WSCADV agrees to: 1. Pay the Contractor a maximum of $24,000 for survivor advocacy services (based on actual hours worked) using the above model, to be reimbursed by WSCADV on a quarterly basis. a. Contractor shall submit invoices to WSCADV prior to the 15th of the month following the end of the previous month. WSCADV shall provide a standard invoice template, to include total hours worked and accompanying rates. Contractor shall submit an activity log listing dates of work, name of employee(s) performing work, hours worked, and rates. Contractor may use standard rates as established by the contracted agency. 2. Pay the Contractor a flat fee of $15,.000 prior to October 31, 2020 to be used for DVHF flexible financial assistance fora minimum of 5 CBHA- Parents As Teachers enrolled families as described above. 3. Pay the Contractor a flat fee of $15,000 (to be used for DVHF flexible financial assistance for a minimum of 5 CBHA- Parents As Teachers enrolled families once the initial $15,000 investment has been fully expended, or on September 30, 2021, whichever occurs first. Any of the total $30,,000 that is unspent at the contract end date shall be refunded to WSCADV. 4. Be responsible for meeting logistics, meetings/trainings, conference calls and webinars. Be responsible for support with financial assistance trackers and reports required based on the information entered into the tracker by the agencies. 6. Provide adequate materials, resources, communication, and support to the first party for the duration of the contract period. In turn, the Contractor agrees to: 1. Perform the tasks outlined below. Collaborate with CBHA - Parents As Teachers home visiting program to implement the partnership that provides mobile advocacy and DVHF flexible financial assistance for families experiencing domestic and sexual abuse who are enrolled in the CBHA - Parents As Teachers home visiting programs. c. With these project funds, pay staff equitably for jobs requiring comparable skill, effort, and responsibility and performed under similar working conditions. d. Provide DHVF flexible financial assistance and advocacy support for a minimum of 5 CBHA - Parents As Teachers families within the first contract year,, and a total of 10 CBHA - Parents As Teachers families within the second contract year. e. Enter the data information into the DVHF tracker for the quarter during the contracted period (due to WSCADV January 7, April 7, July 7, October 7, 2021 and January 7, April 7, July 7, September 7, 2022.) f. Meet the narrative reporting requirements for WSCADV for the OCVA CESS grant. g. Participate in meetings/trainings or conference calls/webinars arranged by the WSCADV. h. Report income from WSCADV in accordance with the laws and regulations of the IRS and other taxing agencies. 2. Provide insurance coverage that shall be maintained in full force and effect during the term of this Contract, as follows: a. Commercial General Liability Insurance Policy. Provide a Commercial General Liability Insurance Policy, including contractual liability, written on an occurrence basis, in adequate quantity to protect against legal liability arising out of Contract activity but no less than $1,000,000 per occurrence and $3,000,000 aggregate. b. Automobile Liability. In the event that performance pursuant to this Contract involves the use of vehicles, owned or operated by the Contractor, automobile liability insurance shall be required. The minimum limit for automobile liability is $1,000,000 per occurrence, using a Combined Single Limit for bodily injury and property damage. c. Fidelity Insurance. Every officer, director, employee, or agent who is authorized to act on behalf of the Contractor for the purpose of receiving or depositing funds into program accounts or issuing financial documents, checks, or other instruments of payment for program costs shall be insured to provide protection against loss: L The amount of fidelity coverage secured pursuant to this Contract shall be $50.,000 or the highest of planned reimbursement for the Contract period, whichever is Page 2 DocuSign Envelope ID: 5COBC937-2791-4DAB-9481-2F52C5023EBC lowest. Fidelity insurance secured pursuant to this paragraph shall name WSCADV as beneficiary. B. Certify that Contractor, defined as the primary participant and its principals, to the best of its knowledge and belief that they: a. Are not presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any Federal department or agency. b. Have not within a three-year period preceding this Contract, been convicted of or had a civil judgment rendered against them for commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public or private agreement or transaction, violation of Federal or State antitrust statutes or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, receiving stolen property, making false claims, or obstruction of justice; c. Are not presently indicted for or otherwise criminally or civilly charged by a governmental entity (Federal, State, or local) with commission of any of the offenses enumerated in paragraph (1)(b) of federal Executive Order 12549; and d. Have not within a three-year period preceding the signing of this Contract had one or more public transactions (Federal, State, or local) terminated for cause of default. 4. Where the Contractor is unable to certify to any of the statements in this Contract, the Contractor shall attach an explanation to this Contract. This contract expires September 30, 2022. This contract may be terminated prior to the given date by either party by a written notice., dated two weeks in advance of the last working day, and stating clearly the cause(s) for termination of the contract.
