REQUEST TO THE COUNTY ROAD DEPARTMENT
FOR REIMBURSABLE WORK
To The County Engineer/ Public Works Director:
Pursuant to terms of an agreement between the County and Adams County
Adopted and approved on March 2 (Name of Requesting Agency)
,2022 the undersigned hereby requests the County to perform the
work listed below at a time and in a manner convenient to the County. The undersigned further requests that the
work be completed no later than Aril 7,2022 and hereby guarantees reimbursement
to the

Cost estimate for Adams Co. bridge repair on Booker Rd:
I estimate that Grant County will take 80 hours to repair the damage on the bridge on Booker Rd.
Repairs include 3 broken corbels and reinstall the bridge posts, repairs to numerous deck
delarnination's.
Expenses:
3 Bridge Crew members at $44.94 each per hour.
AFAD operator $44-94 per hour.
Bridge Service Truck $13.00 per hour.
Flatbed trailer at $17.00 per hour.
Flat Bed Pickup $13.00 per hour.
JLG man lift rental $1301.00 weekly.
3 Kwikbond Easy Patch kits at $74.00 each.
2 Automated Flagging Devices $17.00 each per hour.
Bridge Supervisor at $63.46.
10% Administrative Fee
