WHEREAS, Bill Fode qualifies as a Designated Mental Health Professional as defined
in WAC 388-865-0150 and RCW 71.05 and is employed by Grant County and specifically Grant
Mental Healthcare.
NOW, THEREFORE, BE IT HEREBY RESOLVED by the Board of Grant County
Commissioners that the above named employee be designated to provide the services of a
County Designated Mental Health Professional as defined above effective September 8, 2009.

Grant Mental Healthcare has a current open Crisis Response Specialist position in our
Crisis Department. In order to assist with Crisis coverage Bill Fode is required to be
"designated" to provide ITA services as a County Designated Mental Health
Professional as defined in WAC 388-865-0150 and RCW 71.05.
Bill had been designated by the Board of Commissioners 1990s and then had been
removed from the list of Designated Mental Health Professionals when he became the
Director of Clinical Services.
We wish to have Bill Fode's designation effective September 8, 2009.
If there are other documents that you need provided to you, or if I can be of any other
assistance, please let me lcnow.

DMHP Re-Instatement

I am requesting that 1 have my DMHP status re-instated due to the fact that the Crisis Team will be down one FTE
position for a DMHP and I will need to assisfi the Crisis Team with some of the shifts that need to be covered. I
have been a DMHP for a number of years in the past.
If you could add this request to the BOCC Consent Agenda I would greatly appreciate it. If you have any
