GRANT COUNTY, WASHINGTON
RESOLUTION NO. 2001 -167 -CC
WHEREAS DAN LOPEZ, MA is no longer an employee of Grant Mental Healthcare;
NOW THEREFORE BE IT RESOLVED by the Board of Grant County Commissioners that
the above named employee is no longer designated to provide the services of a
County Designated Mental Health Professional in accordance with WAC 388-865,
WAC 275-55, and RCW 71.05, effective October 15, 2001.
