Tlhniei Aunnt endl]na euttt is made between the Washington State Department of Corrections,
hereinafter referred to as "DOC" or "Department," and Grant County Renew, Grant Behavioral Health and
Wellness, hereinafter referred to as "Contractor," for the purpose of amending the above -referenced
Contract, heretofore entered into between Department and Contractor.
WHEREAS the purpose of this Amendment is to extend the contract term and add a requirement
for Contractor to provide notice of data requests.
NOW THEREFORE, in consideration of the terms and conditions contained herein, or attached
and incorporated and made a part hereof, Department and Contractor agree to the following:
1. Section I. CONTRACT TERM is hereby amended as follows:

2. ATTACHMENT D. DATA SECURITY REQUIREMENTS, is amended by adding the following
to the end of the document:

16. Notice. Contractor agrees to notify Department within two (2) business days of receipt of a
request from, or on behalf of, a federal agency or a federal, state, or local law enforcement
authority, other than the Department, for health care information as defined in RCW 70.02.010,
program eligibility information for individuals, information that may identify a health care
provider's or facility's delivery of health care services to noncitizens, or the delivery of
protected health care services as defined in RCW 7.115.010 where the request may impact
expenditures for such services. Contractor shall give Department adequate opportunity to
seek an injunction or bring other legal action in a court of competent jurisdiction before
granting any requested records.
Additions to this text are shown by underline and deletions by ((Q4ike^���)). All other terms and conditions
remain in full force and effect. The effective date of this Amendment is October 16, 2025.
