Purpose. The purpose of this Amendment is to increase the maximum consideration for services provided and amend Exhibit D — Budget. The Special Terms and Conditions are amended as follows: 2. Exhibit D — Budget is deleted and replaced in full by the revised Exhibit D-1 — Budget and Award Information. All references to the Exhibit D — Budget and Exhibit D — Budget and Award Information are hereby amended to Exhibit D-1 — Budget and Award Information for the period of this Amendment. All other terms and conditions of this Contract remain in full force and effect. Transfer of funds between line -item budget categories of the approved Budget must be requested in writing by the Contractor if the cumulative amount of these transfers exceeds or is expected to exceed $2,000. Budget adjustment requests must be submitted in writing to the DSHS Program Manager using the Budget Revision Request tab of the Invoice Documentation worksheets and are subject to justification and negotiation. Budget line -item revisions approved by the DSHS Program Manager shall be incorporated by reference into this Contract as addendums to Exhibit D-1 and shall not require separate amendments. 2. The Contractor shall provide $59,641 cash or in -kind match for Victims of Crime Act (VOCA) funds payable under the Contract. b. The Contractor shall provide $14,785 cash or in -kind match for Family Violence Prevention and Services Act (FVPSA) funds payable under the Contract. c. Match shall not be from federal funds. All funds designated as match are restricted to the same uses as VOCA and FVPSA federal funds. Match funds may be expended in a greater proportion to DSHS Central Contract Services 6026CF County Program Agreement Amendment (3-31-06) Page 3 contract funds; however, all match funds must be expended prior to the close of this Contract. The Contractor may use state funds payable under this Contract as match toward federal funds payable under this Contract only to the extent necessary to meet the match required by this Contract. Any excess state funds shall not be used as match toward federal funds without the prior written permission of DSHS. If state funds are insufficient to meet the federal match requirement, Contractor shall provide documentation demonstrating additional match as directed by DSHS. 4. Award Information. Funds payable under the Contract are from the following sources: a. $291,753 is allotted under this Contract from the State of Washington general fund. b. $73,927 is allotted under this Contract from the federal Family Violence Prevention and Services Act (FVPSA) grant. The Assistance Listing Number is 93.671. c. $26,510 is allotted under this Contract from federal Family Violence Prevention and Services Act grants awarded through the American Rescue Plan Act of 2021 (FVPSA ARP). This funding must be obligated no later than September 30, 2025 and must be liquidated no later than October 30, 2025. The Assistance Listing Number is 93.671. d. $238,565 is allotted under this Contract from the federal Victims of Crime Act (VOCA) victim assistance grant. The Assistance Listing Number is 16.575. 5. Contractor Assurance Regarding Publications The Contractor hereby agrees that any publications (written visual, or sound), but excluding press releases, newsletters and issue analyses, issued by the Contractor describing programs or projects funded in whole or in part with federal funds under this Contract, shall contain the following statement: "This project was supported by Grant No. 15POVC-23-GG-00478-ASSI awarded by Office for Victims of Crime, US Department of Justice. Points of view in this document are those of the author and do not necessarily represent the official position or policies of the Office for Victims of Crime, US Department of Justice. Grant funds are administered by the Office of Crime Victims Advocacy, Washington State Department of COMMERCE". 6. Corrections to Acknowledgement of Federal Funding In the event a correction is required to the Acknowledgement of Federal Funding, an administrative change will be processed. A change to the Acknowledgement of Federal Funding will not affect the Contractor's budget or scope of work and notice will be provided.
