Engaging in informal monihly check -ins with NCACH staff (in -person or virtual zoom) and Samaritan, These check -ins will serve as status updates allowing for dialogue about successes, ehallenges, and needed support. NCACH staff will contact you to schedule these meetings. 13. Submit reports R.vemi-annually using a template created by NCACH. These brief narrative status updates will encourage -reflection on boih successes and challenges, as well as project -specific metrics. These narrative progress reports will gener all.y b' dUell a. Transformation Project demonstration, -which *Included. a Delivery System Reform Incentive Payment (DSRIP) program, NCACH and the Organization have M* tituall accepted the terms and conditions listed under the Wster Service Agreement (MSA) when registering in the Washington state Financial Executor Portal. The basic agreement language outlined in the MSA apt es to the work completed in this MOO between both organizations, even if the work is not directly tied to a specific D'SRIP project under the Medicaid waiver.
