I am writing to request an extension of -the completion date for the above two SIP grants. They are
both for the see emergency generator and electrical system upgrades project at McKay
Healthcareeering . As you recall, 81P Grant 2021-01 was awarded, for $350,000 to cover professional
W des'gn, development of construction documents, and construction of the base bid
engini
emergency elec%txical generator and electrical system upgrades. I believe you mentioned this grant
has a current completion date M July 2023 6
In June 2022, Public Hospital District No. 4 (d./b`/
a McKay Healthcare) was awarded SIP Grant
No. 2022-02 for additional funding to over the added costs resulting from the unanticipated
effects of the Pandemic and the expanded scope of work associated with the Alternate Bid items.
,According to your recent letter dated June 2, 2023, SIP Project No. 2022-2 has a completion date
of June 21, 2023#
On July 6. 2022, we executed a chain order with the contractor to expand the scope of work to
include the Alternate Bid items and extend the completion date by 231 days to July 4, 2023, This
extension covered the delay M the am*val of -the emergency generator and added time for
construction of the Alteivate Bid items that included electrical system upgrades within the
building. Since then, the contractor has reported that Western. States, its electrical equipment

upp, ier, cannot snip the 1.600 amp switchboard until October 11, 202.3. We are. told the delayvvas

unanticipated because -the manufacturer was not able to keep to 'Its prior commitment to the
supplier. In turn, the supplier could not honor its commitment to the contractor. However, we, are
told that the delivery date of Mid -October, 2023'is a firm date. Because of the added work to
connect conduit and wire after the switchboard arrives, the contractor reports that the project on-
site work may be delayed to January 19, 2024. While we are attempting to work out a completion
date with -the contractor sooner than January 19, 2024, we don"t know if we Will be able to. In the
meantime, we have reached out to local suppliers and to our electrical engineering finn who have
confirmed that the supply chain delays are a real and conn -non problem within the electrical
industry. Our options are iimited.

After substantial completion of the on-site work by January 19, 2024, assuming we stay with the
contractor's lastestimated completion date, there are approximately 60 days at a minimum to
perform the final inspection,. cm�plete punch list items., obtain as -built drawings, complete final
reports, obtain, a Certificate of Completion and Release from the contractor and verify with state
agencies that all subcontractors and employees have been paid. After that is completed final
payment Will be made by the District. Then, the District will be able to request reimbursement
from the Grant County SIP Program. Accordingly, because the two SIP grants 2021-01 and 2022-
02 are for the same pr ject, we are requesting that pr ject compleflon date for each grant be

extended to March 3 2024*
