Purpose
ThisMOU is, an agreement between North Central Accountable Comm n*
u ity of Health
(hereinafter IINCACHI') and Grant Coulnty'DBARenew -hereinafter referred to as the
Organization who partners with NCACH's to provider Recovery Coach services.
The mission of the North Central Accountable Community of flealthis to advance
whole -person health and health equity In North Central Washi*n'gto-n by unifying
stakeholders, supporting collaboration, and driving syst emiic change, with particular
attention to the social determinants of health.
CACis is committed to supporting the work of 'Renew and, the Emergency Department
Recover
,v Coach Program (hereinafter "Program"') to advance our mutual goals.
Specifically, this program involves the delivery of recovery coaching services by Renew
at the Samaritan. Emer ency Department for persons identified as currently using drugs or

alcohol, determined as appropriate for this program and, patient is amehable to

services, recovery education and links to detox, -treatment and recovery community

This Memorandum shall be effective on the date of signing and shall continue -through
December 3 1, 2023, This term may be extended by mutual written agreement of the

For this Memorandum of Understanding, Rene is considered the co -lead organization for
the attached scope(s) of work (Exhibit A), along with Samaritan Healthcare

responsible for completion and submission of deliverables listed below and those outlined
in the scope of work displayed in Exhibit A.

A. Engaging in informal monthly check -ins with NCACH staff (in-person or virtual
z . oom) and Samaritan. These check -ins will serve as status updates allowing for
dialogue .a .bout successes, challenges, and needed support. NACH staff will contact

B. Submit reports wmi-annually using a template created by NCACH. These. brief
narrative status updates will encourage reflection on both successes and challenges, as
well as project -specific metrics. These narrative progress reports will generally be
due;
a. On the I 5hday of the month after the first six months of work.
b. Every six months thereafter.

IV, Responsibilities of NCACtI

V. Chante in Status
In the event of substantive change in the legal status, organizational structure
or fiscal

NCACH of the change. The parties shall ide notice as soon as practicable but no later

VI. Amendment of tb,e Memorandum

VI L Termination ofthe Memorandum
Either party may terminate this Memorandum upon 30 days'prior written, notification. to
the other party.
VIII. Costs
Unless otherwise specified within this Memorandum, all expenses incurred by the
participants of this collaborative project are the responsibility of`th. participant,

IX. Master Service Agreement
The finds for this MO U were earned through Washington State's Medicaid
Transformation Project demonstration, which included a Delivery System Reform
Incentive Payment (DSRIP) program. NC and the organization have mutually
accepted the terms and conditions listed tinder the Master Service ► reement (MSA)
when registering in the Washington State Financial Executor Portal. The basic
agreement language outlined in the MSA applies to the work completed in this MOU
'between both or, even if the work 'I's not directly tied to a specific DSRIP
Project under the Medicaid waiver.
