WHEREAS, Juan Padilla, Shasta Sitton, John Hoyer, Mark Hoffman, and Diane
Truscott qualify as a Designated Mental Health Professional as defined in WAC 388-865-0150
and RCW 71.05 and are employed by Grant County and specifically Grant Mental Healthcare.
NOW, THEREFORE, BE IT HEREBY RESOLVED by the Board of Grant County
Commissioners that the above named employees be designated to provide the services of a
County Designated Mental Health Professional as defined above effective September 1, 2010.

Re: Designating Juan Padilla, Shasta Sitton, John Hoyer, Mark Hoffman, and Diane Truscott
Dear Commissioners:
Grant Mental Healthcare would like to have Juan Padilla, Shasta Sitton, John Hoyer, Mark
Hoffman, and Diane Truscott designated to provide the services of a County Designated Mental
Health Professional as defined in WAC 388-865-0150 and RCW 71.05. These employees are
current Therapists with Grant Mental Healthcare, who would like to be available to provide on-
call Crisis Services.
Each Therapist has a Master's Degree and has worked with us for two or more years. They are
all recognized by the State of Washington as Mental Health Professionals.
We wish to have Juan Padilla, Shasta Sitton, John Hoyer, Mark Hoffman, and Diane Truscott's
designations effective September 1, 2010.
If there are other documents that you need provided to you, or if I can be of any other assistance,
please let me know.
